  1. About This Service

    The Young Adults Transition Service at Monash Health assists young adults with complex medical needs and/or developmental disabilities to transition from paediatric to adult healthcare services. The client’s physical, emotional, social and vocational needs are considered when planning a coordinated transition.

    The healthcare team includes: • Medical specialist • Occupational therapist • Physiotherapist • Social worker • Neuropsychologist • Hand therapist • Wheelchair specialist • Allied health assistant • Speech pathologist

    A care plan may include: • Home, vehicle or wheelchair modifications • Wheelchair and equipment prescription • Exercise program • Counselling • Community living skills • Upper limb management • Swallowing and communication assessments • Medical assessment • Cognitive assessment • Information on community services

      • Eligibility Criteria

        • Young adults aged between 16 and 25 years with complex medical needs/development disability who require coordinated transition from paediatric service to adult services • Living in Victoria • With a complex medical need and/or developmental disability • Require two or more allied health professionals (such as physiotherapy, social work etc.) • Where a coordinated transition from paediatric to adult healthcare services is required

      • Participation Length

        The client will be discharged from the service when the transition care plan has been completed.

      • Participation Type

        A tailored transition care plan is developed with the client, their carers and the multi-disciplinary healthcare team. The healthcare services required will depend on the client’s goals.

      • Specific Exclusions

        Young people aged under 16 years or those over 25 years of age.

      • Who Can Refer

        An allied health or medical professional can refer to this service.

      • Referral Mode

        A medical referral letter including a comprehensive patient history and goals for transition are required.
