    PIR (Partners in Recovery) provides care coordination for people with severe and persistent mental illness who have complex needs across different services. We help to coordinate your care and work with numerous agencies to build safety nets for our target clients. PIR has a lead agency called the PHN but partners with six external organisations who employ 'support facilitators'.

      • Eligibility criteria

        People who have a severe mental illness such as schizophrenia or bi-polar affective disorder that is considered complex and long lasting. Complex means that you need help from more than one agency to get the care you require. Long lasting usually means the illness has been present for many years, or is expected to remain so.
